TITLE: HOME HEALTH AIDE-HOSPICE TEAM
PRIMARY PURPOSE OF UNIT: Home health service is that component of comprehensive health care whereby services are provided to individuals and families in their places of residence for the purpose of promoting, maintaining, or restoring health, or minimizing the effects of illness and disability.
PRIMARY PURPOSE OF POSITION: To perform personal care activities, household tasks and errands (only with certain payer sources) as outlined in Home Health Aide care plan. The care plan is to be developed and supervised by a Registered Nurse.
EDUCATION: High School diploma or equivalent and C.N.A. certificate.
EXPERIENCE: Minimum of 6 months to 1 year recent (within 24 months) C.N.A. experience in a hospital or nursing home facility required.
CERTIFICATION: C.N.A. certificate from a program which meets current State of Maine minimum requirements. Currently active and listed in good standing on the State of Maine C.N.A. Registry.
LEARNING PERIOD REQUIREMENT: Orientation through Agency based program.
OTHER JOB REQUIREMENTS:
Must have a valid Maine driver's license
Proof of automobile insurance
Must have a reliable automobile
Must submit CNA Certificate & currently be on the Maine State Registry
Must re-train in CPR every two years
