PRIMARY PURPOSE OF UNIT: The Central Intake Department functions as the main coordinating department for all patient care referral requests, medical orders, facility discharge planning requests, private insurance reimbursement authorization and case management, monitoring patient telehealth data as well as responding to external patient care information requests regarding agency services.
PRIMARY PURPOSE OF POSITION: To document and coordinate referral information from hospitals, long-term care and nursing facilities, physicians, community agencies, patients and families necessary for patient admission into agency clinical care programs. Serve as a liaison nurse contact for discharge planning in response to the needs of referral sources and patients in need of agency services.
EDUCATION: Graduate of professional school of nursing with state and regional accreditation required. Certificate in case management desirable. BS degree in nursing preferred.
EXPERIENCE: Two years recent nursing experience in home health care, community- based health care setting required. Desirable assets include strong computer, communication and organizational skills.
LICENSURE AND CERTIFICATION: Current RN license in the State of Maine. ANA certification in Home Health or Community Health Nursing desirable.
LEARNING PERIOD REQUIREMENTS: Successful completion of an Orientation Program in the Intake Department.
OTHER JOB REQUIREMENTS:
Must have a reliable automobile.
Must have current Maine drivers license.
Independent malpractice insurance strongly advised.
Evidence of automobile insurance.
Must submit current RN license every two years to Human Resources Dept., on or before the expiration date.
