PRIMARY PURPOSE OF DEPARTMENT: The Central Intake Department functions as one main coordinating unit for all internal and external referrals for all agency programs and provides information to customers regarding Agency services.

PRIMARY PURPOSE: To function as a Liaison between referral sources & the Intake department, to manage Intake referrals with the support of an RN for clinical related information as needed; to perform central telephone reception, data entry, and general office tasks associated with Central Intake Department support. To assist in agency P.I. programs as needed.

EDUCATION: High School Diploma required. Graduation from a two-year business college with an Associates Degree preferred. Medical terminology preferred.

EXPERIENCE: 2-3 years of broad office experience. Basic computer knowledge, data entry experience required, and working knowledge of medical terminology and healthcare insurance authorization processing preferred. Experience in dealing with internal and external customers in a fast-paced multi-task environment required.


Work as a team member with Intake Nurses and Supervisor by screening of all incoming office phone calls, assuring that
calls are directed appropriately and initiating the collection and data entry of demographic patient referral information.
Process referrals not requiring medical orders as directed by the Supervisor.
Electronic medical record tracking of to-do lists including NTFC & eval-only visits, pre-admit & hold.
Maintain patient records in accordance with established protocols and policies.
Support for regulatory initiatives (ie: F2F Encounters).
Liaison with area hospital, physicians, & Facilities to meet the Intake requirements.
Initiate calls to referral patients, recently opened & recently discharged patients.
Perform other functions and tasks to promote the smooth operation of Intake Department and coordination of operations with other departments.
Coordinate management of Pharmacy Assistance Program.
Coordinate communication and documentation related to patient transfers to in-patient facilities.
Distribute all Intake Department incoming and outgoing mail and inter-departmental communications.
